Description

American Traveler is seeking a compassionate and skilled Physical Therapist for a 13-week contract at a Skilled Nursing Facility, with experience in SNF or geriatric rehab settings preferred.

Details

• Skilled Nursing Facility (SNF) setting, focused on inpatient, long-term care, and short-term rehabilitation
• Patient population includes geriatric residents requiring mobility restoration, functional improvement, and independence optimization
• Day shift schedule: 9:00 AM – 5:00 PM
• 13-week contract

Requirements

• Graduate of an accredited Physical Therapy program required
• Current, active state PT license required; license required if applicable
• CPR/BLS certification preferred
• Experience in a SNF or geriatric rehab setting preferred
• Strong communication, documentation, and interpersonal skills required
• Ability to perform physical tasks including lifting, standing, walking, and assisting patients with transfers and mobility
• DOB and SSN may be required for consideration

Additional Information

• Responsibilities include evaluating residents' physical conditions, developing individualized treatment plans, and delivering evidence-based rehabilitative interventions
• Collaborate with an interdisciplinary team including physicians, nursing staff, occupational and speech therapists, and family members
• Monitor and document patient progress in accordance with state, federal, and Medicare/Medicaid guidelines
• Educate patients and caregivers on safe mobility techniques, home exercise programs, and adaptive equipment use
• Supervise physical therapist assistants (PTAs) and aides as needed
• Participate in care planning meetings and interdisciplinary team conferences
• No COVID-19 vaccine requirement for this position
