This is a Temporary Promotion/Detail Not to Exceed One (1) Year, it may be terminated at any time. It may be extended or converted to permanent without further competition. Any extension of the temporary promotion/detail will be communicated in writing and subject to the same eligibility and documentation requirements as the original appointment. The Patient Safety Officer oversees the VISN patient safety program, serving as a key member of the Quality Management and High Reliability teams. The role includes monitoring, analyzing, and trending patient safety events; leading the VISN patient safety committee; supporting facility Patient Safety Managers; and ensuring compliance with VA policies and regulatory requirements. The incumbent provides subject matter expertise in patient safety, human factors, just culture, RCA processes, and evidence-based safety methodologies. Responsibilities include coordinating recall processes, reviewing JPSR and RCA data, monitoring follow-up actions, and disseminating lessons learned, best practices, and safety strategies across the network. The Patient Safety Officer acts as the VISN liaison to the National Center for Patient Safety, leads training and awareness efforts, and guides innovative approaches to improving care delivery. The position collaborates with the QMO, CMO, and facility leadership to identify system vulnerabilities, recommend improvements, and support a transparent, high-reliability culture. The incumbent also participates on VISN and VHA committees, assists with survey readiness, and oversees mandatory reporting, HFMEA, Institutional Disclosure, Tort claims, and Peer Review-related patient safety requirements. Central to the role is identifying system-level risks, developing and implementing preventive actions, and ensuring VISN-wide patient safety standards are met to reduce harm to Veterans and staff.
