Assists with policy and procedure development and modifications as neededEducates, staff, managers and leaders of assigned communities on quality improvement processesWorks directly with assigned community leadership teams to monitor indicators, dashboards and performance improvement projects. Tracks, analyzes and reports on all PIPs to Quality teamAssists assigned communities with implementing, monitoring and tracking of resident, family and employee satisfaction surveysImplements and monitors community action plansFacilitates performance initiatives at assigned communities and assists with plans of correction as neededComplete quality indicator tracking and trending reports. Support community leaders on quality initiativesMonitor compliance in accordance to Heritage Senior Living policies, best practice protocols, and state and federal regulatory requirementsProvide education to staff on electronic health record system to ensure compliance with documentation standardParticipates in and monitors new company programs and pilots as necessaryAct as interim DON depending on the needs of the organizationManage the training and the onboarding of DONsAssist with the Corporate Onboarding which includes, but is not limited to orientation & CBRF trainingsPartners with the Vice President of Clinical & Quality Operations to develop, implement and evaluate annual infection prevention goals and action planPartners with facility leaders, physicians, local, state, and national agencies on activities related to infection preventionOversees the operations of the infection prevention, epidemiology, industrial hygiene, and relevant safety programsAccountable for surveillance of healthcare acquired and community acquired infectionsEnsures that education and counseling on infection prevention is available for staff, volunteers, medical staff, patients and parents.Education/QualificationsValid state of Wisconsin RN licensureMust be in good standing on the State Board of Regulation and LicensingThree years of nursing experience working with seniors; experience within an assisted living, memory care or other long-term care environment preferredMust be self-directed and able to function independentlyManagerial experience preferredExcellent interpersonal skills – ability to connect with residents, families, and associatesStrong customer service orientation to older adults, exhibit a caring and compassionate attitudeAbility to make responsible choices and decisions that are in the best interest of the residents and caregiversAbility to work collaboratively with all coworkersExcellent written and verbal communication skills; must be proficient in Microsoft; PointClickCare software experience a plusAbility to problem solve and communicate complex issuesMust submit to all applicable pre-employment and annual tests per state and federal regulationsMust exhibit a caring and compassionate attitude while articulating true concern for peopleEssential FunctionsTo perform this job successfully, an individual must be able to perform each essential duty.
