Title
Case Manager - Hospice Nursing

Schedule
Tuesday - Saturday, 8am - 4pm - On-call rotation required.

Assignment Details
Guaranteed Hours: 37
Contract Weeks: 13

Requirements
- Valid nursing license
- Knowledge of OASIS documentation
- Proficiency in High-Tech nursing skills (IV use, wound care, etc.)
- Experience in case management and patient assessments

Roles & Responsibilities
- Assess patient and caregiver needs to provide high-quality care in accordance with evidence-based practice and regulations.
- Develop and communicate realistic plans of care that involve patients and caregivers.
- Manage case loads effectively while ensuring all documentation is timely and accurate.
- Implement nursing interventions and care plans, ensuring infection control and adherence to standards.
- Educate patients and caregivers about treatment options, medication side effects, and discharge instructions.
- Collaborate with interdisciplinary teams to coordinate comprehensive patient care.

Compliance / Notes
- Compliance with agency protocols and documentation processes is required.
- Participation in peer review audits and case conferences is expected.
