Social Worker (Program Coordinator) - Outreach

Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Applicants must meet eligibility requirements per VHA policy and submit an EDRP application within four months of appointment. Award amounts up to $200,000 and eligibility periods of one to five years are determined by the VHA Education Loan Repayment Services program office after a complete review. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. United States Citizenship: Non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master’s degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master’s degree in social work. Verification of the degree can be made by visiting to verify that the social work degree meets the accreditation standards for a master of social work. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Loss of Licensure or Certification: Social workers must maintain a full, valid, and unrestricted independent license or certification; otherwise they are removed from the GS‑0185 social worker series and may be terminated. May qualify based on the Grandfathering Provision described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Social Worker (Program Coordinator) – GS‑12 experience and education. One year of experience equivalent to the GS‑11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post‑master’s degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinators must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills and Abilities: Knowledge of program coordination and administration, including consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Candidate has demonstrated experience conducting independent clinical outreach services in non‑traditional settings such as street outreach, tent encampments, shelters, or other community‑based locations. Candidate has experience providing independent diagnostic assessments, evidence‑based treatment, and case‑management services to individuals with co‑occurring mental health disorders, substance use disorders, and chronic homelessness in settings with limited supervision. Candidate has established or maintained collaborative relationships with community agencies or other stakeholders to coordinate resources and supportive services to homeless individuals. Duties The social worker is responsible for providing independent outreach‑based case management throughout the state. Louis VA catchment area. This position is 70% patient care and 30% program coordination and administration. Leading and developing strategic outreach in response to available data related to homelessness in collaboration with community agencies and within the VA system. Managing local outreach calls, overseeing data for local response to the National Call Center for Homeless Veterans. Providing and monitoring the department’s after‑hours homeless outreach phone and providing on‑call services as assigned. Developing collaborations with community agencies serving the homeless and conducting independent street outreach to engage Homeless Veterans. Organizing and monitoring micro and mezzo veteran outreach to homeless and at‑risk veterans within assigned areas. Developing recurring and responsive outreach for assigned areas. Leading strategic planning, including real‑time safety data analysis and program response for homeless veteran outreach. Leading the homeless veteran outreach team in performance improvement and sustainment activities to optimize team efficiency and outreach services for veterans. Monitoring service mapping and duplication of service through comparable programs such as SSVF. Providing independent clinical outreach services at the full extent of the advanced practice clinical level within privileges, credentials, scope of practice, elements of practice, certification, functional statement, and position description. Completing community diagnostic assessments in non‑traditional settings for those veterans unable to access traditional in‑office clinics. Developing communication platforms with team members and community agencies to convey clinically sensitive information and ensuring compliance with VA privacy standards. Developing community‑based outreach activities to target tent encampments and other identified hotspots that require homeless veteran engagement appropriate to geographic locations. Deploying research‑based strategies for safe and effective rural and inner‑city settings, data entry and reconciliation with HMIS and other community homeless data systems. Maintaining a complex clinical caseload of veterans identified as vulnerable and in need of advanced practice community‑based clinical services. Performing other duties as assigned. Work schedule: Monday to Friday, 8:00 am to 4:30 pm. Compressed or flexible schedule not available. Recruitment incentive: No sign‑on bonus.
