Position Overview

We are seeking a skilled healthcare professional with home care experience for a position in skilled home care services. The role involves providing high-quality medical care to patients in the comfort of their homes.

Job Details

• Shift: Day 5x8-Hour (08:00 AM - 05:00 PM)
• Location: Local candidates accepted in the vicinity of City, State

Key Responsibilities

• Conduct cardiopulmonary assessments and basic wound care management.
• Administer IV therapy, wound care, and manage complex diagnoses such as heart failure, COPD, myocardial infarction, pneumonia, stroke, and post-operative care.
• Perform home health visits, approximately 6-8 per day, self-scheduled around office needs.
• Manage the following tasks:
  
  
  • ABI compression dressing
  • Pleurx/Asept drain care
  • Foley catheter change and care
  • Staple/suture removal
  • Use of glucometer and pro-time machine
  • Subcutaneous and intramuscular injections
  • Phlebotomy and lab draws
  • PICC line or Midline care including dressing changes
  • Access and deaccess SQ ports

Qualifications

• 1 year of skilled home care experience required. Private duty nursing experience alone is not accepted.
• Experience with OASIS training and documentation is essential.
• Current certifications: BCLS (Basic Cardiac Life Support).
• Must have a valid driver's license and access to a car for home visits.
• Recent patient care experience (within the last 3 months) is mandatory.
• Must be COVID-19 vaccinated or have a valid exemption.
• Flu shot is required, or a declination must be on file; mandatory mask-wearing while at work is necessary.

Additional Information

• Ability to use a laptop and MiFi for internet access during home visits.
• Portable printers may be provided for documentation purposes.
• The role requires a team-oriented approach with interdisciplinary support from Physical Therapy, Social Services, Pharmacy, and Interpretation Services.

Patient Demographics

• Patient age groups include adults and geriatrics.
• Daily census at various locations ranges from 400 to 1000 patients.
