Responsibilities The Social Worker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients. They provide patient‑ and family‑centered care, manage multiple priorities, and engage in creative, compassionate, and ethical problem‑solving. They perform comprehensive assessments and develop treatment plans that integrate medical, social, and resource issues, collaborating with the multidisciplinary health team to implement patient‑centered care plans. Essential Functions Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and environmental issues. Formulates biopsychosocial assessments, dispositions, and treatment plans. Collaborates with other professionals to evaluate patients' medical or physical condition and assess client needs as part of the interdisciplinary team. Monitors, evaluates, and records client progress according to measurable goals described in the treatment and care plan. Qualifications Master’s Degree in Mental Health & Behavioral Medicine or Master’s Degree in Social Work. Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW), or Licensed Mental Health Counselor (LMHC). At least 2 years of clinical experience in a medical setting. Additional Knowledge, Skills And Abilities Proficient clinical judgment and application of professional values and ethics. Skilled in formulating risk assessments, crisis intervention, psychosocial assessments, treatment plans, and timely documentation. Advocates for clients while maintaining professional boundaries. Knowledge of internal and external resources and ability to refer appropriately. Provides evidence‑based interventions. Coordinates and communicates treatment plans with patients, families, and the interdisciplinary team. Social sensitivity and ability to engage in complex organizational contexts. Utilizes supervision and consultation as needed. Seeks assistance with complex cases and situations in a timely manner. Engages in quality improvement projects and uses data to measure progress. Facilitates skill‑based groups. Communicates findings to a broader audience. Schedule and Work Model Full‑time, Monday through Friday, approximately 8 am–4:30 pm; flexibility with hours possible. Primarily remote with possible travel in the community. Meetings at Assembly Row in Somerville, MA twice per month. On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Teams Video is required and must be accessed using MGB‑provided equipment.
