Position Overview

This position is for a Home Health Registered Nurse (RN) with a focus on providing high-quality patient care in a home health setting. The assignment typically spans 13 weeks, with a standard work schedule from Monday to Friday, 8 AM to 5 PM. There may be occasional weekend rotation, approximately once every 4 to 6 weeks.

Location

Working territory will include the following region: Westchester, NY.

Key Responsibilities

• Conduct home visits to assess patient needs and deliver appropriate care.
• Document patient interactions, care plans, and follow-up notes in accordance with established protocols.
• Maintain effective communication with patients, families, and interdisciplinary healthcare teams.
• Attend periodic office meetings as required.
• Manage a caseload of 5 to 8 patients daily, with weekly productivity expectations of approximately 30 points.
• Travel between patient homes, with a weekly travel estimation of 200 to 250 miles.

Qualifications

• A minimum of one year of recent experience in adult home health or hospice care.
• Certification in CPR and BLS is required.
• Proficiency in Homecare Homebase (HCHB) EMR, with recent usage within the past six months, or completion of a retraining course.
• Experience with OASIS assessments is mandatory for RNs.
• Reliable transportation, a valid driver's license, and agency-required liability insurance are essential.

Work Environment

This role requires the nurse to travel to patient homes for daily visits as well as to a clinic for weekly meetings. Candidates must be open to on-call responsibilities and flexible scheduling. There are no guaranteed hours, and all documentation must be completed within 24 hours of patient appointments.
