Spanish Cove At Home in Yukon, OK is seeking an experienced RN Hospice Case Manager to deliver compassionate, patient-centered end-of-life care within the Home Hospice department. You will develop and implement individualized hospice plans, coordinate care with the Interdisciplinary Team, and support families through difficult transitions. The role requires an Oklahoma RN license, BLS/CPR, and at least 1 year of med-surg/geriatric or long-term care experience; supervisory experience is #J-18808-Ljbffr Spanish Cove
