Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Maintain accurate, timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
• Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay
• Participate in student clinical education programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
• Promote a culture of patient and employee safety, adhering to safe patient handling practices, infection control protocols, and all clinic safety policies

Required Qualifications

• Active Physical Therapist license in good standing in the state of New Hampshire
• Compliance with all state-mandated continuing education requirements
• Adherence to the discipline-specific State Practice Act and applicable regulatory agency standards
• Ability to perform in an ethical manner consistent with the PT professional code of conduct

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience with clinical student supervision and mentorship
• Familiarity with performance improvement methodologies and quality initiatives
