About the Position

Licensed Social Worker – Acute Hospital

Schedule: Monday–Friday, 8:00 AM–4:30 PM
Weekend Rotation: Rotating weekends. When working a weekend day, the Social Worker will receive a weekday off.

Position Overview

We are seeking a Licensed Social Worker with strong acute hospital experience to support patients and families with discharge planning and care coordination. The ideal candidate will have hands-on experience working within Med-Surg and Emergency Department (ED) units .

Key Responsibilities

• Provide comprehensive discharge planning and transition-of-care support.

• Assess patients' psychosocial needs and identify barriers to safe discharge.

• Coordinate post-acute services, including appropriate community resources and follow-up care.

• Collaborate with physicians, nurses, case managers, and other members of the interdisciplinary care team.

• Communicate effectively with patients and families regarding discharge plans and available resources.

• Maintain accurate and timely documentation in the medical record.

• Assist with care coordination and facilitate appropriate transitions from the acute-care setting.

Required Qualifications

• Active Licensed Social Worker (LSW) license in New Jersey.

• Previous acute hospital experience is required.

• Experience working with Med-Surg and Emergency Department (ED)
