MDS Coordinator Restorative Director RN or LPN

This is a dual role and the candidate will be both the MDS Coordinator and the Restorative Coordinator. The candidate must be proficient in MDS and experienced in restorative therapies. This opportunity is open to an RN licensed in Illinois who will assume primary responsibilities for care planning and MDS completion within the facility, facilitate conference meetings, perform rehabilitation assessments on all new admissions and for existing patients quarterly and as needed, and ensure each patient receives individualized restorative care. Responsibilities Setting up and maintaining an organized system for determining and scheduling Care Plan/MDS review dates. Notifying each department of conference date, time, location, and specific residents to be reviewed. Maintaining a record of attendance at meetings by all staff members. Calling the meeting to order and acting as timekeeper throughout, ensuring that discussion is pertinent and the meeting ends on time. Conducting the meeting efficiently and effectively, allowing each discipline the opportunity for input and questions, and ensuring that input is reflected in the MDS/Restorative plan. Eliciting necessary and appropriate information from staff members to promote the development of problem/need statements, goals, and approaches. Responsible for notifying families to invite them to care‑plan meetings. Ensuring that departments are aware of changes in a resident’s conference. Relating information to or from the conference in the absence of staff members. Ensuring that all changes to MDS and Restorative Plans are made in a timely manner. Transmitting MDS assessments within guidelines outlined by Medicare and Medicaid. Informing the administrator of any staff absence from conference. Submitting copies of each transmittal report to the Administrator/DON. Discussing changes to MDS with each discipline as they relate to their area before changing codes. Performing other duties as assigned by the Administrator or DON. Performing or overseeing functional evaluations on admission, quarterly, and as clinically indicated. Consulting with nurses to make recommendations for restorative plans of care. Working with nurses in developing restorative plans of care. Overseeing that all restorative nursing measures are reflected on the care plan as an approach to the focus or needs being addressed. Working closely with the therapy department. Ensuring that specific recommendations from therapy are carried through after discharge. Reviewing weekly documentation completed for residents receiving restorative services and ensuring that all proper ADL documentation has been completed for patients. Overseeing incontinent management programs. Attending care‑planning conferences and making recommendations. Reporting progress of restorative nursing measures. Attending Interdisciplinary/QAPI meetings as indicated to provide feedback and identify any residents for evaluation of restorative service needs. Coordinating with Human Resources and Director of Nursing to orient new staff to restorative nursing principles. Coordinating with Director of Nursing and therapy to provide ongoing education/evaluation of direct‑care staff on restorative nursing principles. Actively participating in physical restraint reduction and fall‑management programs. Setting up documentation flow sheets for daily documentation of restorative nursing measures for each resident in coordination with medical records. Performing or supervising the work of restorative nursing staff and serving as liaison with all therapists (physical, occupational, speech). Participating in patient care planning and CMI meetings. Working Conditions and Qualifications The role involves working closely with residents, staff, visitors, and government agencies. The RN must handle hostile and emotionally upset residents and families with patience, tact, and enthusiasm, maintaining a calm and positive environment. The position requires standing, stooping, bending, lifting, and walking intermittently throughout the workday, with the ability to lift at least 25 lbs, transfer, and turn residents. Qualifications Registered Nurse (RN) licensed in the State of Illinois. Long‑term care experience. Familiarity with the functions of each discipline involved in resident care and care planning. Effective communication and leadership skills, with a strong sense of organization. Computer proficiency. #J-18808-Ljbffr
