Check all med changes, D/C, new meds on MAR correctly, labels correct, and old meds in discard box. Check ALL renewal Rx against MAR entryPrepare Outgoing Consults-VS, Menses, seizure…check against calendar for upcoming weekWrite Nursing Progress Notes-note PRN use, health issues being monitored, overall health, medication effect. File-Ensure Manager has reviewed all the consults or signed the Medical Appointments Week of…for that week and then file completed consults in the Individual's medical recordReminders: When notified of med change…ensure it arrives, plan to red/black dot later and remove old labels****Also answer triage calls, follow-up medication refills, in addition to fulfilling the Weekly/Monthly/As Due responsibilitiesWeekly (Initial all documents weekly in red)Medication Inventory-RefillsMAR- note and address -missing signatures -new/changed medications (check Rxs to see started on time, entry correct, label correct, old meds in discard box) -refusals -comments on back of MAR -PRN use -ensure each signature for B/P, P, wt, FS, etc. Has documented valuesEmpty Blisterpacks-ensure all doses accounted forControlled Counts-do a count yourself and ensure it is correctDispose Medications –within 72 hours if controlled, within a week if notHealth Stats-VS in range, FS in range, seizures, weights, Menses, Bowel charts, Glucometer memory-check against FS log
