Work Environment

This full-time position offers an onsite work environment, primarily serving patients in their homes as part of our home care initiative. You will work a consistent schedule from Monday to Friday, 8 am to 5 pm, allowing for structured work-life balance while fulfilling the high demand for services in your community. The role involves conducting in-home visits, developing individualized treatment plans, and collaborating with other healthcare professionals to ensure comprehensive patient care.

Key Responsibilities

• Evaluate and assess patients' physical conditions, establishing personalized treatment goals based on their unique needs and circumstances.
• Administer a patient load of 1 individual per hour, providing focused attention and tailored interventions.
• Develop comprehensive treatment plans utilizing various therapeutic techniques, aiming to restore and enhance patients' range of motion and mobility.
• Design customized fitness and wellness programs, incorporating strength training, flexibility exercises, and balance training specifically aimed at improving patients' functional capabilities.
• Monitor and document patient progress meticulously, ensuring clear communication with both patients and the broader healthcare team.
• Promote a proactive and positive environment, encouraging patients to engage actively in their recovery through motivation and support.
• Implement cost-effective therapies that minimize reliance on medication and advocate for non-surgical alternatives whenever feasible.
• Collaborate with physicians and other healthcare professionals to ensure continuity of care and optimal patient outcomes.

Patient Population / Client Focus

Qualifications

• A Master's or Doctorate degree in Physical Therapy (MPT or DPT) from an accredited institution.
• A valid Florida Physical Therapist license with no recorded board or disciplinary actions.

• Proficiency in utilizing electronic medical record (EMR) software such as WebPT for efficient patient documentation.

Preferred Experience

• Previous experience as a Physical Therapist in orthopedic outpatient settings is highly desirable.
• Demonstrable experience working in home care environments is a plus.

Compensation and Benefits
