Registered Nurse

Job Title: Registered Nurse Job Location: Brentwood, TN Job Shift: Full Time Sign on Bonus: $10,000 Position Overview The RN provides and directs nursing care to patients in their homes as prescribed by the physician, complying with applicable laws, regulations, and agency policies. Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization. Maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources. Conduct initial and/or comprehensive nursing evaluation visits, determine primary focus of care, develop the plan of care within state-specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer. Maintain and submit accurate documentation and clinical notes, including patient condition, goals, interventions, and response to care. Establish and maintain an effective and efficient visit route. Provide high-quality clinical services within the scope of practice defined by state laws, in accordance with the plan of care, from admission through discharge. Must Haves One (1) year of clinical RN experience. Current RN license in the state where services are rendered. Current CPR certification. Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation; reliable commute to multiple patient residences daily within the assigned geographic area.
