Overview
Physical Therapist (General) in Franklin, NH. Start date: 07/20/2026. Travel - temporary assignment lasting 13 weeks with guaranteed 35 hours per week. Weekly pay range: $2,350–$2,479 per week. Shift: 8:00 AM–4:30 PM. Setting: Skilled Nursing Facility. This is a travel assignment arranged by Healthforce Healthcare LLC.

Responsibilities
- Perform initial PT evaluations for residents and develop individualized plans of care.
- Implement therapeutic interventions including therapeutic exercise, gait training, balance work, transfers, and padding/assistive devices as appropriate.
- Monitor progress, adjust treatment plans, and document outcomes according to regulatory and facility standards.
- Collaborate with an interprofessional team (nursing, therapy colleagues, physicians) to optimize mobility, function, and discharge planning.
- Educate residents and families on home exercise programs, safety, fall prevention, and activity modification.
- Ensure patient safety, maintain infection control, and comply with documentation and regulatory requirements.

Requirements
- Current, active Physical Therapist license in New Hampshire.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Experience in skilled nursing facility or geriatric rehabilitation preferred; strong clinical skills in mobility, ADL training, and rehab modalities.
- Current CPR/BLS certification preferred.
- Ability to work in a skilled nursing setting and collaborate within an interdisciplinary team.
