RN Coordinator Home Health/Hospice -Continuing Care Intake

We are currently seeking an experienced Registered Nurse to join our Continuing Care Intake team as a Home Health/Hospice Coordinator. In this role, the Coordinator will support patients from UNC Medical Center in Chapel Hill, collaborating with clinical case managers and the multidisciplinary team to initiate and coordinate home health services with our post‑acute care team. The ideal candidate will have a minimum of two years of nursing experience, with a strong foundation in Medical‑Surgical nursing. Prior experience in Home Health nursing or Case Management is a plus. Schedule & Benefits Full‑time position scheduled Monday through Friday from 8:00 a.m. to 5:00 p.m., with some on‑call responsibilities. Responsibilities Communicate all appropriate patient information and coordinate staff availability directly with the home health or hospice intake department. Coordinate with the patient, patient’s family, referring physician, clinical care management representative, and hospital staff to determine the appropriateness of all home care referrals. Educate referral sources and other hospital staff regarding home health or hospice services and regulatory requirements. Gather all required demographic and medical information, as well as treatment and services requested, to develop a detailed and comprehensive home health or hospice care referral and plan. Identify the type of service that best serves the referral patient (General Inpatient Hospice or Home Hospice) and communicate this to the referral source and patients/families/care givers. Education & Experience Graduation from a state‑accredited school of professional nursing. BSN preferred. Two years of home health, public health, or other health‑related experience. Licensed to practice as a Registered Nurse in the state of North Carolina. Two years of nursing experience with a foundation in Medical‑Surgical nursing. Licensure & Certification Licensed to practice as a Registered Nurse in the state of North Carolina.
