Hospice Registered Nurse

Overview Full Time or Part Time Hospice Registered Nurse. This role supports patients and families with hospice care, providing emotional support and care to help patients live their final days with dignity and comfort. The Hospice RN conducts supervisory visits for clinical observation of assigned team members and participates in interdisciplinary group meetings to develop, review, and revise the patient/family plan of care. Responsibilities Provide on-going assessment of patient physical, functional, psychosocial, and environmental needs Implement and revise individualized plans of care Ensure compliance with AdvisaCare Hospice Policies and Procedures and Medicare requirements Educate patients and families on disease processes, self-care techniques, end-of-life care, and ethical concerns Supervise paraprofessionals providing services to patients Prepare clinical and progress notes and participate in IDG conferences Participate in Hospice performance improvement programs Maintain high levels of customer satisfaction with clients, families, and caregivers Communicate information using current processes and technology within the organization Requirements Current Michigan license in good standing Minimum 1 year hospice experience Empathetic approach to patients and caregivers Participate in field visits daily and weekly office meetings as indicated Ability to adapt to patient status and needs on a full-time on-call basis Additional Details On-Call responsibilities required Location: West Branch, MI and surrounding area; Charlevoix Office location as parent agency Employment type: Full-time and Part-time options We are a growing hospice agency seeking compassionate, dynamic, energetic, organized, and efficient professionals to join our team. #J-18808-Ljbffr
