Job DescriptionClinical Pharmacist \n\nLocation: Terre Haute, IN & Clinton, IN\n\n$50k Sign On Bonus!!!!!\n\nMedSys is actively recruiting for a Clinical Pharmacist for a direct-hire opportunity with a leading regional healthcare organization. This position offers the opportunity to work in a collaborative clinical environment focused on improving patient outcomes through evidence-based medication management and interdisciplinary care.\n\nResponsibilities\n\n Review and verify medication orders to ensure safety, accuracy, and appropriate therapeutic outcomes.\n Provide clinical pharmacy services, including drug information, medication therapy recommendations, and dosing consultations.\n Collaborate with physicians, nurses, and care teams to optimize patient care and medication management.\n Participate in antimicrobial stewardship, formulary management, and quality improvement initiatives.\n Monitor patient therapy, review laboratory results, identify potential drug interactions, and recommend treatment adjustments.\n Provide patient counseling and education regarding medication therapies and disease state management.\n Support pharmacy operations through medication verification, sterile and non-sterile compounding, and technician oversight.\n Serve as a clinical resource and participate in staff education, training, and committee activities.\n\n\nAmbulatory Care Focus (as applicable)\n\n Partner with primary care providers to support chronic disease management programs.\n Manage patients with diabetes, hypertension, hyperlipidemia, COPD, CHF, asthma, and other chronic conditions.\n Conduct medication reviews, patient education, and therapy monitoring to improve clinical outcomes.\n Participate in anticoagulation management and medication reconciliation activities.\n\n\nQualifications\n\n Doctor of Pharmacy (PharmD) required.\n Active pharmacist license or ability to obtain licensure.\n Residency training and/or previous clinical pharmacy experience preferred.\n Strong knowledge of medication therapy management, pharmacotherapy, and clinical best practices.\n Excellent communication and collaboration skills.\n Commitment to providing exceptional patient-centered care.\n\n\nWhy Apply?\n\n Direct-hire opportunity with a respected healthcare organization.\n Collaborative team-oriented environment.\n Opportunities for professional growth, continuing education, and clinical development.\n Meaningful work that directly impacts patient outcomes and quality of care.\n\n\nInterested candidates are encouraged to apply through Medsys/CSI Companies for consideration.
