About This Role The RN Case Manager is responsible for assessing, planning, coordinating, monitoring, and evaluating healthcare options and services to meet individual patient needs. Highlights Pay: $43.38 - $74.94/Hourly Location: Northampton, MA Facility: Cooley Dickinson Hospital, Inc. Schedule: Full-Time, Rotating What You'll Do Assess individual health care needs across the continuum. Plan and coordinate appropriate levels of care and post-acute care needs. Monitor and evaluate options and services for patients. Collaborate with physicians and staff to determine care requirements.
