Location: Fremont, CA area Facility Type: Post-Acute / Skilled Nursing Schedule: Monday–Friday, 35 hours per week No Weekends Pay: $54–$59 per hour Employment Type: Permanent, Full-Time Facility Size: 199 beds Job Summary Talented Medical Solutions is seeking a compassionate and skilled Occupational Therapist to join a supportive post-acute care team in the Fremont, CA area . This is a full-time permanent opportunity offering a steady Monday–Friday schedule with no weekends . The Occupational Therapist will evaluate patients, develop individualized treatment plans, and provide therapy services that help patients improve independence, daily living skills, strength, coordination, and overall functional ability. Responsibilities Evaluate patients and develop individualized occupational therapy treatment plans Provide skilled therapy services to improve daily living skills, strength, mobility, coordination, balance, and independence Monitor patient progress and adjust treatment plans as needed Collaborate with nursing, therapy, and care teams to support patient goals Educate patients, families, and caregivers on therapy goals, safety, and functional strategies Complete documentation accurately and timelyFollow all facility, state, and federal guidelines Qualifications Graduate of an accredited Occupational Therapy program Skilled nursing, post-acute, rehab, long-term care, or geriatric experience preferred Strong clinical judgment and documentation skills Excellent communication and teamwork abilities Compassionate, dependable, and patient-focused License and Certification Requirements Active California Occupational Therapist license required NBCOT certification required CPR/BLS certification required Must be eligible to work in a skilled nursing/post-acute care setting Compensation and Schedule $54–$59 per hour 35 hours per week Monday–Friday Permanent full-time position #J-18808-Ljbffr TEEMA Solutions Group
