ID/DD Case Manager - Coordinated Care & Family Support

A non-profit organization in Virginia is seeking a qualified ID/DD Case Manager. This full-time position requires a BA/BS in Psychology, Social Work, or a related field, along with experience in Human Services. The Case Manager will conduct assessments and service linkages for consumers with ID/DD, provide education and follow-up support, and ensure compliance with all relevant standards.
