Wound Care Nurse Position Summary

This is a full Time Mon-Fri position. Occasional weekends may be required

standard hours are 7a-330p

The Wound Care Nurse is responsible for assessing, treating, and monitoring patients with acute and chronic wounds. This role provides specialized nursing care to promote wound healing, prevent complications, and improve patient outcomes. The Wound Care Nurse works collaboratively with physicians, nurses, therapists, caregivers, and other members of the healthcare team to develop and implement individualized wound care plans.

Essential Duties and Responsibilities

• Perform comprehensive assessments of wounds, including size, depth, drainage, tissue type, surrounding skin, and signs of infection.

• Develop, implement, and evaluate individualized wound care treatment plans.

• Provide wound treatments, including dressing changes, debridement as permitted by scope of practice, negative-pressure wound therapy, and other prescribed interventions.

• Monitor wounds for healing progress, infection, deterioration, and other complications.

• Recommend appropriate wound care products, dressings, and treatment modalities based on patient needs and provider orders.

• Collaborate with physicians and other healthcare professionals regarding wound management and changes in patient condition.

• Educate patients, families, and caregivers regarding wound care, skin protection, nutrition, pressure injury prevention, and signs of infection.

• Provide guidance and education to nursing staff regarding evidence-based wound care practices.

• Assist with the prevention and management of pressure injuries, diabetic wounds, venous ulcers, arterial ulcers, surgical wounds, and other complex wounds.

• Maintain accurate and timely documentation of assessments, treatments, interventions, and patient progress.

• Participate in quality improvement initiatives related to wound prevention, treatment, infection control, and patient safety.

• Ensure wound care practices comply with facility policies, professional standards, and applicable regulations.

• Maintain knowledge of current evidence-based wound care practices, products, and treatment techniques.

• Perform other nursing duties as assigned.

Qualifications

• Current, Registered Nurse (RN) license

• experience in wound care, medical-surgical nursing, long-term care, home health, rehabilitation, or a related clinical setting preferred.

• Current BLS/CPR certification.

• Strong clinical assessment, communication, organization, and documentation skills.

• Ability to work effectively with patients, families, healthcare providers, and interdisciplinary teams.
