Behavioral Health Case Manager

The Behavioral Health Case Manager coordinates and ensures effective communication among team members, facilitators, and families. This full time position will require work from home and in person work, with required local travel within Sheboygan County or Manitowoc County Wisconsin. We will prioritize individuals with case management backgrounds, management backgrounds, and education in an applicable field. This position does not require a license, but may require further training during onboarding

Job Title: Behavioral Health Case Manager

Location: Sheboygan County or Manitowoc County, Wisconsin

Position Type: Full-time (combination of work from home and in-person work)

Salary: $45,000 - $50,000 per year

Job Description:

The Behavioral Health Case Manager at The Production Farm will be responsible for coordinating and ensuring effective communication among team members, facilitators, and families. This position requires a Bachelor's degree in Social Work or a related field, with at least 2 years of experience in case management and management. A Master's degree in Psychology, Social Work, or Health Care Administration is preferred.

Responsibilities:

- Attend team meetings and assist facilitators in establishing goals for service plans

- Document team meetings and progress on treatment goals

- Conduct tours with new Consumers and their families

- Communicate with facilitators and parents regarding consumer behaviors and concerns

- Coordinate care with Consumer's treatment team

- Manage adult CCS clients caseloads and coordinate with adult facilitators

- Assist in scheduling individual skill building clients and group curriculum

- Support staff as they facilitate groups and adult programming

- Assess effectiveness of groups and adult programming

- Other duties as assigned

Qualifications:

- Bachelor's degree in Social Work or related field

- Master's degree in Psychology, Social Work, or Health Care Administration preferred

- At least 2 years of experience in case management and management

- Ability to collaborate and communicate effectively in a team setting

- Experience with screening for mental health and substance abuse disorders

- Knowledge of evidence-based psychosocial treatments and interventions

- Basic knowledge of psychopharmacology for mental health disorders

- Driver's License required
