Position Summary Physical Therapist – Outpatient (Staff). Location: 401 15th St SE DeMotte, IN 46310. Employment type: ongoing staff. Schedule: Monday‑Friday, two evenings per week. Base salary: starting at $76,000 annual base, commensurate with experience. Incentives: potential $5,000 sign‑on bonus and student relief program. Responsibilities Complete and maintain all corporate and clinical services required training and maintain compliance with state/local/federal regulations.
