Wellness Specialist

Position Overview The Wellness Specialist provides services and consultation as a member of a mobile multi-disciplinary treatment team (ACT team). Responsibilities Participate in daily treatment review meetings. Provide support to all specialists on the team, addressing overall consumer wellness. Serve as backup to the substance abuse specialist when absent. Provide support around services related to wellness, medication management, health, appointments, and individualized goals. Document services provided, particularly in the development of service plans. Ensure revisions to the plan as the participant’s needs change. Advocate for the rights and preferences of the participant. Develop wellness interventions, activities, and groups, drawing from the Wellness Self-Management curriculum and other resources; educate participants about self‑help techniques, processes, and community options; teach coping strategies based on personal experience. Assist participants with symptom‑management skills and help clarify rehabilitation and recovery goals. Assist in developing community support systems and networks. Consistently practice according to recovery‑oriented and trauma‑informed service delivery principles. Assess and provide services addressing health, wellness, housing, income support, education, vocational training, employment, and social supports; complete an initial comprehensive recovery‑oriented service plan and a 6‑month plan review incorporating participant and relevant others’ feedback. Provide ACT treatment services including service planning and coordination, problem solving, support with housing, social connections, family relationships, independent living skills, resource acquisition, education and training access, employment supports (job search, placement, and support), entitlement and financial management, empowerment and self‑help, wellness self‑management with a focus on coping skills, medication support, and weekly groups. Use motivational interviewing, recovery and trauma‑informed approaches when delivering services; complete crisis intervention and relapse prevention plans; collaborate with participants, families, natural supports, shelter, and community partners to promote attainment of treatment and recovery goals. Coordinate discharge in collaboration with inpatient and ER staff when participants have involvement. Complete thorough, timely, and accurate documentation and authorization records consistent with billing, regulatory, policy and contractual requirements. Complete expected minimum monthly treatment contacts, with 80% occurring in the community. Provide 24‑hour crisis intervention on‑call services on a rotating basis. Required Qualifications Master’s degree in social work, psychology, rehabilitation counseling, or a related field. Excellent verbal, written, and interpersonal communication skills. Preferred Qualifications One year of experience working with adults with severe mental illness. Bilingual in any language (plus not required). Basic computer skills (e.g., MS Office suite and Windows). Program‑Specific Details ACT is an intensive, community‑based multidisciplinary treatment team that provides person‑centered care to a shared caseload of adults whose needs cannot be met through traditional outpatient services.
