Registered Nurse - Home Health

Job Description

Job Summary:
Provides comprehensive home health nursing care to patients in accordance with physicians' orders and agency policies. Coordinates patient care activities and supervises home health aides and licensed practical nurses to ensure quality outcomes.

Responsibilities:

• Assess patient needs and develop individualized care plans based on clinical evaluations.
• Administer medications and perform wound care, documentation, and other clinical procedures.
• Supervise and evaluate home health aides and LPNs/LVNs to ensure compliance with care plans.
• Document patient visits and clinical notes accurately within required timeframes using HomeCare HomeBase.
• Travel up to 75 miles from the home clinic to provide care at patient homes, managing schedule flexibility.
• Communicate effectively with the clinical team and attend periodic office meetings.

Qualifications:

• Valid RN license in the state of practice, in-hand at the time of application.
• Minimum 1 year of recent adult home health or hospice experience, depending on the specialty.
• Proficient in OASIS and HomeCare HomeBase; retraining may be required if used outside the last 6 months.
• Reliable transportation, valid driver's license, and insurance are mandatory.
• Ability to travel up to 75 miles per day and accept all scheduled patients regardless of distance.
