RN Case Manager - Entry Level

Role Overview This position requires decisive judgment, strong clinical assessment, and the ability to build positive relationships. The incumbent will lead and support healthcare teams, providing education and resources to facilitate effective patient care, utilization of services, appropriate level of care, and safe transitions for patients and families. Key Responsibilities Lead care coordination efforts, ensuring adherence to medical necessity criteria and discharge planning in acute or emergency care settings. Assess patient needs through critical thinking and organizational skills, making informed decisions. Collaborate with internal and external stakeholders via effective communication. Support compliance with federal and state utilization review and appeal requirements. Qualifications & Skills Minimum of 3 years' RN experience, preferably with discharge planning in acute/emergency settings. Knowledge of medical necessity criteria and utilization review processes. Bachelor of Science in Nursing preferred; Certification of Case Management (CCM) is a plus. Strong interpersonal, written, and verbal communication skills.
