Work Environment This position is based onsite and involves providing therapy services directly within our clients' homes. You will work closely with a compassionate team of healthcare professionals, including physical therapy assistants, to foster a supportive atmosphere that prioritizes patient rehabilitation. Collaborating with other healthcare disciplines, you will assess and address the unique needs of each client, further enhancing their rehabilitation experience. Key Responsibilities Direct comprehensive physical therapy treatment aimed at preventing disabilities, alleviating pain, and maximizing functional abilities, all aligned with prescribed care plans. Supervise and provide guidance to physical therapy assistants, ensuring they deliver high-quality care consistent with established treatment protocols. Conduct thorough evaluations of clients' functional abilities using diagnostic assessments and clinical tests. Assess and make recommendations regarding the home environment to facilitate rehabilitation and enhance client independence. Implement prescribed physical therapy services in-home, using evidence-based practices to administer therapeutic exercises and treatments. Utilize manual therapy techniques, as well as administer traction and massage, to address various patient conditions. Evaluate, fit, and potentially modify orthotic and prosthetic devices based on client needs and physician directives. Collaborate in developing, reviewing, and updating individual care plans for clients, adjusting as necessary based on ongoing evaluations. Document patient progress meticulously and communicate findings with the client's physician and case managers. Educate clients and families on their therapy programs and the use of therapeutic devices, fostering engagement and understanding. Participate actively in interdisciplinary team meetings to strategize comprehensive treatment plans and share valuable insights on client progress. Maintain accurate clinical records including initial evaluations, care plans, and discharge summaries as per regulatory requirements. Respond to any changes in client conditions by making necessary adjustments to care strategies, in coordination with healthcare providers. Patient Population / Client Focus As a Physical Therapist at Resurgens Physical Therapy, you will work with a diverse patient population that includes individuals recovering from surgery, conditions related to aging, neurological disorders, and various musculoskeletal injuries. Your expertise will be vital in providing tailored rehabilitation plans that honor each client’s unique circumstances and goals, thus aiding in their recovery and promoting long-term wellness. Qualifications A Doctorate in Physical Therapy (DPT) from an accredited program. Active Physical Therapist licensure in the state of Virginia, with no history of sanctions or disciplinary actions. Demonstrated mastery of manual therapy techniques and exercise prescription. Proficiency with electronic medical record (EMR) systems, particularly Web PT, to ensure accurate documentation. Current knowledge of physical therapy practices supported by clinical research. Preferred Experience Prior experience in a home care or outpatient rehabilitation setting is highly desirable. Ability to work independently and adapt to various client environments effectively. Experience collaborating with interdisciplinary teams in client-centered care approaches.
