Psychiatric Mental Health Nurse Practitioner (PMHNP) – Outpatient

A growing, mission-driven Federally Qualified Health Center (FQHC) in Western New York is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join its collaborative Behavioral Health team. Your Role

Provide outpatient psychiatric evaluations and medication management
Diagnose and treat mental health conditions across the lifespan
Develop individualized treatment plans
Collaborate with therapists, primary care providers, and care coordinators
Educate patients and families on treatment options
Participate in quality improvement and team-based care initiatives

Qualifications

Board-Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
Active New York NP license (or ability to obtain)
DEA preferred
Outpatient psychiatry or community health experience preferred
