Responsibilities:

• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities
• Develop and implement individualized care plans in collaboration with the healthcare team
• Participate in staff meetings and contribute to a positive and respectful work environment
• Communicate effectively with residents, families, and healthcare team members
• Maintain accurate and timely records of patient care and progress

Requirements:

• Previous experience as a Licensed Practical Nurse (LPN) preferred
• Licensed Practical Nurse (LPN) License
• 1 years of experience or higher
