Responsibilities In collaboration with the physician and preceptor, implement the assessment plan, evaluate, and follow up on patient care, ensuring holistic consideration of physical, emotional, social, educational, and functional needs. Interpret assessment data to develop a nursing care plan that includes problem identification, nursing diagnosis, nursing orders, approaches, interventions, short/long‑term goals, and discharge planning. Participate as a productive member of the interdisciplinary team in assessment, planning, implementation, and evaluation of rehabilitation goals. Ensure that documents in the medical record are complete, factual, accurate, and timely. Minimum Qualifications Graduate of an accredited nursing program. Valid state RN license required.
