Peacehaven is searching for a compassionate individual to support adults with Intellectual and Developmental Disabilities (I/DD) for the role of Qualified Professional (QP).

 

QPs at Peacehaven are responsible for overseeing caseloads for service recipients, day to day services for members assigned to their caseload, and supervision of Direct Support Professionals (DSPs). QPs assist adults with I/DD work towards their individual goals and support their integrated participation in all aspects of daily life. The ideal candidate for this position integrates the position's responsibilities in an organic way that fosters autonomy and growth. This role requires trust, collaboration, flexibility, and communication to ensure all members of the Peacehaven community thrive in their learning, working, and playing together.

 

The IDEAL candidate would have Care Management experience within the MCO environment, or an equal amount of knowledge within the IDD system of care for 1915i and Innovations Waiver services. The role includes periodic after-hours availability for member meetings and emergencies as needed.

 

Performing The Job:

· Adhere to Peacehaven standards and best practices, designed to ensure a healthy culture that embodies Peacehaven’s Mission, Vision, and Core Values.

· Document progress of individual(s) supported, incidents, strengths, needs, and other relevant matters via our Electronic Heath Record (EHR), OnTarget, and maintaining other electronic documentation and communication on a daily basis.

· Communicate clearly and effectively with family members, work associates, and community members to ensure a positive environment for all.

· Work in an immersive and integrated community to support growth and new skill-building while supporting a healthy lifestyle.

· Comfort, celebrate, motivate and encourage individuals you are supporting and other team members.

· Primary daily duties, and responsibilities include, but are not limited to,

• • Developing the Short Range Goals and monitoring its implementation.
  • Contributing to and verifying the accuracy of the ISP, and monitoring its implementation.
  • Leading and/or facilitating the planning meetings for the development of an individual’s PCP.
  • Arranging, coordinating, linking, and referring individuals to appropriate services.
  • Training and supervising Direct Support Professionals (DSP).
  • Reviewing DSP service notes for timeliness (24-hour submission standard) and accuracy (every note during a DSP’s first 60 days, spot-checks thereafter).
  • Scheduling and creating documentation to support treatment plans.
  • Providing DSP supervision at the required frequency (monthly or quarterly depending on the service), including the new-hire onboarding schedule and field observations, with a signed supervision note each session.
  • Conducting member-care supervision (Supervision of Members) on the same monthly/quarterly cadence.
  • Coordination of care: monthly check-ins with each member’s family or legal representative and documenting contacts with external providers (day program, employment, medical, behavioral health, Care Manager, MCO).
  • Completing incident reports and meeting IRIS reporting timelines by level (Level I, II, and III); serving as a mandated reporter for abuse, neglect, or exploitation.
  • Participating in and serving as an informant for member assessments (CMCA, SIS, NC-SNAP) and the PCP/ISP planning process.
  • Maintaining required tracking tools and ensuring any rights restrictions are documented, reviewed on schedule, and brought to the QA/QI (human rights) Committee.

· Working with staff, volunteers, and families to ensure that the least restrictive environment is available to help individuals grow and flourish and their rights are honored.

· Participating in a variety of meetings including Peacehaven staff and training meetings, community life Meetings, and other meetings as scheduled.

· Embodying the mission and vision of Peacehaven by modeling the core values.

· Undertaking other duties as assigned by members of leadership.

 

Knowledge, Skills, and Abilities:

· Ability to model and encourage healthy behavior for individuals in social situations, program participant relationships, facilitating healthy lifestyle choices, and maintaining program space cleanliness.

· Effective oral and written English communication skills.

· Desire to work in an environment where varied beliefs, spiritual, and philosophical backgrounds are respected and encouraged.

· Maturity as demonstrated by the ability to work cooperatively as part of a team.

· Ability to learn about intellectual disabilities, mental health, health disorders, simple nursing care, behavior management strategies, personal hygiene, etc.

· Ability to complete and retain necessary training and documentation, with a willingness to obtain additional training (CPR, First Aid, HIPAA, Med Admin, Bloodborne Pathogens, etc.).

· Honor the confidentiality of participants, records, practices, and business efforts.

· Working knowledge of the documentation systems used at Peacehaven: OnTarget (EHR), SharePoint, HHAeXchange (EVV), and the state IRIS incident portal.

· Understanding of Medicaid service authorizations, service definitions, and billing units, including the consequences of documentation and authorization errors.

· Familiarity with the NC regulatory framework (10A NCAC 27G, NCGS 122C) and the HCBS Settings Rule.

· Knowledge of person-centered planning and trauma-informed care principles.

 

Minimum Requirements:

· Qualified Professional: 1) A master’s degree in a human services field and 1 year full time supervised experience in the field, 2) a bachelor’s degree in a human services field and 2 years of full time, supervised experience in the field, or 3) a bachelor’s degree in a field other than human services and 4 years of full time supervised experience in the field.

· Have a working knowledge of Innovations Waiver, 1915i Waiver, and Intellectual Developmental Disabilities.

· Documentation requirements per the Records Management and Documentation Manual (RMDM), July 2025 edition.

· Ability to adhere to, train, review, and maintain Departmental Policy and Procedural needs.

· Ability to work as a member of a team and non-profit.

· Valid driver’s license and ability to drive comfortably within a 50 mile radius dependent upon their caseload needs.

· Discrete and confidential in handling private information.

· Ability to pass background checks.

· Applicants need to have healthy strategies for self-care and physical well-being.

· Must be drug free.

· Able to work in a tobacco-free environment.
