Job Title: Travel Occupational Therapist (Home Health) Location: Wayne County, Troy, Utica, and surrounding coverage areas (Oakland, Livingston, Washtenaw, Genesee, and Macomb Counties) Schedule: Monday-Friday | 8:00 AM - 4:30 PM
On-Call/Weekends: None

Productivity: 24 points/week Position Summary Seeking an experienced Travel Occupational Therapist to provide in-home therapy services, evaluate patient needs, develop individualized care plans, and help patients achieve maximum independence and safety in their home environment. Key Responsibilities
- Conduct OT evaluations and develop individualized treatment plans.
- Perform admissions/comprehensive assessments when nursing services are not ordered.
- Provide ADL/IADL training and therapeutic interventions.
- Educate patients, families, and caregivers.
- Conduct home safety assessments and recommend adaptive equipment.
- Complete reassessments and maintain accurate documentation.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Graduate of an accredited Occupational Therapy program.
- Current unrestricted Occupational Therapist license in the state of practice.
- Current CPR/BLS certification.
- Prior home health experience required.
- Experience with OASIS documentation, home health regulations, Conditions of Participation (CoPs), functional assessments, fall prevention, ADL/IADL training, and care coordination.
- Proficiency with HomeCare HomeBase (HCHB).
- Strong home health background.
- Advanced HCHB experience.
