MLTC Care Manager & Care Coordinator

CVS Health in New York seeks an Analyst, Care Manager to provide comprehensive care management to MLTC plan members. You will conduct psychosocial assessments, develop person-centered service plans, and coordinate long-term services and supports.
The role requires NY LMSW/LCSW licensure, 2+ years in care management, strong communication and collaboration skills, and willingness to travel within the service area.
