Trinity Home Health Care

Are you a compassionate and dependable Registered Nurse looking for a flexible schedule and the opportunity to make a meaningful difference in patients' lives? Trinity Home Health Care is seeking an experienced Per Diem RN to join our growing team.
Responsibilities:

Complete Start of Care (SOC) and admission assessments
Perform routine supervisory visits for home health aides
Provide skilled nursing visits in patients' homes
Develop, implement, and update individualized care plans
Collaborate with physicians, caregivers, and interdisciplinary team members
Complete accurate and timely electronic documentation
Educate patients and families on disease management, medications, and home care

Qualifications:

Current Registered Nurse (RN) license
Minimum of one year of nursing experience (home health experience preferred)
Excellent assessment and communication skills
Reliable transportation and valid driver's license
Current CPR certification
Ability to work independently and manage a flexible schedule
