SUMMARY

A Critical Time Intervention (CTI) Nurse plays a vital role in delivering support to individuals in the critical transition periods, such as moving from institutional care (like hospitals or shelters) to community settings. The Critical Time Intervention Nurse will work as part of a multidisciplinary team to provide nursing care and support for individuals during critical transitions. The role involves addressing physical health needs, mental health concerns, and coordinating care to ensure a seamless transition from institutional settings to community living. The CTI Nurse will collaborate closely with case managers, social workers, and other healthcare professionals to ensure clients receive comprehensive support.

RESPONSIBILITIES & DUTIES

The duties and responsibilities of the position are to be carried out in a manner that is consistent with the mission, values, and operating principles of Jewish Family Services.

• Work closely with the CTI team to develop individualized care plans for each client.
• Ensure clients have access to necessary medical services, such as follow-up appointments, prescriptions, and referrals to specialists.
• Conduct comprehensive health assessments for clients, including physical, mental, and behavioral health evaluations.
• Assist clients with accessing community healthcare resources, including doctors, therapists, and medical facilities.
• Facilitate referrals for additional services, such as addiction treatment, housing assistance, or mental health support, as needed.
• Educate clients on managing chronic health conditions, medication adherence, and lifestyle changes to support their well-being.
• Offer guidance on maintaining health and self-care, focusing on preventive care.
• Empower clients to make informed decisions about their health and treatment options. Accurately document all interactions with clients and all efforts made towards client engagement. 
• Provide clinical support during crisis situations, including medication management and stabilizing physical or emotional health crises.
• Act as a resource for clients in times of stress or uncertainty, offering immediate nursing interventions or referrals.
• Collaborate with social workers, case managers, and other healthcare professionals to ensure a holistic approach to client care.
• Participate in regular team meetings to discuss case progress, share insights, and address client needs.
• Assist with developing discharge plans and ensuring clients are connected to necessary supports in the community.
• Submit all progress notes within 48 hours of the client encounter. 
• Complete necessary documentation for legal, health, and social service purposes, ensuring compliance with privacy regulations (e.g., HIPAA).
• Report any changes in client condition to appropriate team members and healthcare providers.
• Advocate for clients' healthcare needs, ensuring that they receive adequate medical care and mental health support.
• Support clients in navigating the healthcare system, including insurance or Medicaid-related issues.
• Effectively utilize electronic systems including electronic documentation and reporting systems. 
• Attend agency and department in-service training and staff meetings as well as any other agency related activities as required.

QUALIFICATIONS Education and Experience:

• Bachelor’s degree in nursing (BSN) required; associate’s degree in nursing (ADN) may be considered with additional experience. 
• Must have active New York State licensure. 

• Minimum of 2-3 years of nursing experience, preferably in community health, mental health, or social services.
• Experience with the Critical Time Intervention model or similar case management interventions is a plus.

Knowledge, Skills & Abilities: 

• Strong clinical assessment and nursing skills.
• Knowledge of medication management, including psychotropic medications.
• Knowledge of mental health, substance use, housing, and social service systems.
• Ability to work under pressure in high-stress situations, with strong crisis intervention skills.
• Excellent communication, interpersonal, and crisis management skills.
• Experience with case management and client-centered care models.
• Ability to analyze and use data to improve program delivery.
• Familiarity with case management software and documentation systems

• Exhibited ability to effectively work within an inclusive and culturally and linguistically diverse environment.
• Strong internet research and computer skills, especially with Outlook, Word, Excel, PowerPoint, and web-based health information systems.
• Bilingual skills desirable. English speaking and writing fluency required.

WORKING CONDITIONS

• Will work in the office and in the community; able to travel outside the office to various sites to attend meetings and provide support services.
• Must have access to a reliable vehicle, possess a valid, clean driver’s license and be sufficiently self-insured with liability insurance in the amount of $100/$300k.
• Flexible hours including days and some evenings and/or weekends.

PHYSICAL REQUIREMENTS

• Physical activities and efforts required working in an office environment.
• Visual acuity sufficient to maintain system of files and reports containing computer-generated and handwritten documents. 
• Auditory acuity sufficient to communicate with staff, clients, and others by phone and in person.
• Mobility sufficient to conduct regular duties within a normal office environment and community.

COMPENSATION & BENEFITS

• Competitive salary of $65,000 - $70,000 annually, commensurate with experience & qualifications.

• Accrued Paid Time Off (PTO) of 4+ weeks in addition to a paid agency wide shut down between Christmas and New Year's Day.
