Licensed Practical Nurse

Licensed Practical Nurse / Licensed Vocational Nurse This role offers a unique opportunity to advance your nursing career in a setting that provides high‑impact work. You will deliver nursing services, training, and support to address the medical or psychiatric needs of adults with brain injury, intellectual and developmental disabilities, substance use disorders, and/or emotional or behavioral challenges. Responsibilities Monitor the health conditions of individuals served and ensure quality services are provided. Administer medications, document administration, and oversee staff who administer medications. Provide prescribed medical treatments per physician order and perform personal care services. Conduct assessments, implement health‑care services for each Individual Service Plan (ISP), and respond to changes in condition. Provide staff training on universal precautions, infection control, and medication administration. Serve as liaison and advocate with medical and professional providers, and coordinate services with external agencies. Educate individuals served on self‑medication, prescribed treatments, sex education/STD prevention, and smoking cessation. Qualifications Associate’s Degree in Nursing or related field, or state certificate. At least one year of experience working with the population served in a nursing capacity. Current state LPN or LVN licensure for the state of residence. Current driver’s license, car registration, and auto insurance. Required certifications such as CPR/FA, HIV/AIDS certification, BBP, and ACT training. Strong communication skills with staff, individuals served, and their families.
