Community Health Navigator
Scenic Bluffs Community Health Centers
Scenic Bluffs Community Health Centers is hiring multiple full-time Community Health Workers to support clinics in Cashton and surrounding areas. CHWs help patients identify and overcome non-clinical barriers, navigate social services, and coordinate with care teams to ensure services are accessed.
Strong communication and outreach skills are essential. The role emphasizes ongoing navigation, outreach, education, and follow-through for patients with barriers, with travel to assigned clinic
