CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver in-home therapy, under the supervision of a Physical Therapist, to help patients regain strength and independence in their own homes. Responsibilities include implementing treatment plans, performing therapeutic exercises and modalities, documenting records weekly, and coordinating with the team and branch leadership.
