Role: Licensed Practical Nurse (LPN) providing direct patient care in home settings. Responsibilities Deliver high-quality clinical services within legal scope, following the care plan and infection control protocols. Thoroughly document patient visits in electronic medical records and electronic visit verification systems. Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members. Adapt to diverse home environments, regardless of patient demographics. Qualifications & Skills Current, unrestricted LPN licensure in the practice state. Valid driver’s license, vehicle insurance, or access to public transportation.
