LPN Charge Nurse Apple Rehab Avon, located in Avon, CT, is a 60‑bed family‑oriented skilled nursing and short‑term rehabilitation facility. Starting Pay: Competitive rates up to $37 per hour, based on experience. Differentials Offered Shift Differentials for 2nd and 3rd shifts and weekends Additional Shift Differential: Extra $5.00/hour for shifts worked outside scheduled hours Available Shift 24‑hour 3‑11 PT position Responsibilities Plan, execute, and oversee nursing services for assigned residents, adhering to quality care standards, facility policies, legal requirements, medical practices, and physician instructions. Efficiently plan and provide professional care aligned with residents’ needs and the changing medical landscape. Maintain professionalism in interactions with residents, physicians, families, peers, and support staff. Document crucial information related to resident care accurately and comprehensively. Administer prescribed medications and ensure accurate record‑keeping in line with facility policies. Requirements Valid state nursing license in good standing (or in the process of becoming a registered nurse). In‑depth knowledge of nursing practices, medical procedures, and long‑term care regulations. Strong attention to detail, excellent organizational skills, and ability to manage multiple tasks effectively. Collaborative team player with previous experience in long‑term care preferred. Additional Expectations All LPN positions require weekend on‑call availability to support staff and residents. Per diem staff are expected to fulfill two weekend shifts monthly.
