Physical Therapist - $10,000 Sign-On BonusSierra Health Care has served Las Cruces families since 1982. As a Physical Therapist, you'll bring skilled, compassionate therapy into patients' homes, helping them move safely, regain independence, and stay where they feel most at home. You'll be based in Las Cruces, with occasional travel to Truth or Consequences as needed. This role fits a therapist who values clinical independence and works well with an interdisciplinary team.Key ResponsibilitiesEvaluate patients and develop, carry out, and update individualized plans of care aligned with physician ordersDeliver skilled interventions and maintenance therapy programs within your state scope of practiceComplete initial evaluations, progress notes, reassessments, and recertifications in line with Medicare and state standardsPrepare and submit OASIS assessments and other required documentation on timeTeach patients and caregivers home exercise programs, mobility strategies, and safety techniquesSupervise Physical Therapist Assistants and Home Health Aides in line with state practice actsConsult with physicians and team members on treatment adjustments and patient progressTake part in QAPI activities and share your expertise through staff in-servicesOther Duties:Provide care and services in accordance with infection control, safety, and risk prevention protocolsAccurately document clinical interventions, care coordination, and communication in a timely mannerParticipate in interdisciplinary meetings, case reviews, and continuing education opportunitiesMaintain compliance with HIPAA and all company privacy and confidentiality standardsRepresent the organization positively and professionally in patients' homes and community settingsWork independently with minimal daily oversight and demonstrate initiative in completing responsibilitiesProvide courteous, timely, and professional customer service to patients, families, and partnersExhibit adaptability to changing responsibilities and flexibility in completing assignmentsDemonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platformsAdhere to all organizational policies, including job descriptions, mission, and the Employee HandbookCommunicate and interact professionally and respectfully with others to support team goalsPerform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the companyWork EnvironmentPrimarily works in the fieldSupervisorClinical Manager - Home HealthSupervisesPhysical Therapy AssistantRisk LevelPatient care or physical work with injury riskRequirementsWhat We're Looking ForA graduate of an accredited physical therapist programA current Physical Therapist license in the state of practiceTwo years of clinical experience preferred; home health experience is a plus but not requiredSound clinical judgment and comfort working independently in varied settingsStrong communication skills and sensitivity to diverse backgroundsBasic computer skills and a willingness to learn our EMR and OASIS documentationCurrent CPR certificationA valid driver's license, reliable transportation, and auto insurance for home visitsPhysical RequirementsMust be able to stand, walk, bend, and assist with mobility or equipment as needed.
