General Summary/Responsibilities:Status: Full-time: *40 Hours per week (80 hours per pay period), variesDepartment: COMPREHENSIVE OP REHAB PROGRAMFacility: Miami Valley HospitalThe medical social worker provides direct social work intervention, transition planning and counseling to patients and their families. The medical social worker assesses the patient’s social, emotional, financial, cultural and family situation and interprets this information to the physician, nursing staff and other care team members to facilitate appropriate transition planning. The medical social worker assists patients and families in making appropriate linkages to community resources and provided consultation to those seeking input and guidance in efforts to resolve patient concerns. Transition planning and social work services are provided to inpatient and outpatient populations of the hospital.Qualification:EducationMinimum Level of Education Required: Master's degreeArea of study or major: Social WorkLicensure/Certification/RegistrationLSW required; LISW preferredLicensure with the State of Ohio Counselor, Social Work and Marriage and Family Therapist BoardExperienceMinimum Level of Experience Required: 1 - 3 years of job-related experiencePreferred experience: Previous hospital or community social work experience required. Knowledge of community agencies preferredKnowledge/SkillsExcellent interpersonal skills, critical thinking, and ability to communicate in writing.Excellent organizational skills.Ability to work in a busy, stressful environment. #J-18808-Ljbffr
