Physical Therapist

Position Overview Physiotherapist – Home Care. Schedule: 8:00–5:00 PM, every 4th weekend on‑call. Schedule & Compensation Base salary plus an uncapped bonus structure; $20,000 sign‑on bonus. Qualifications & Attributes Licensed physical therapist in the state of employment Minimum one year experience as PT in an acute setting Desirable: home care experience Excellent communication and positive outlook Knowledge of therapy best practices and ability to make sound judgments Detail‑oriented, disciplined, and timely Valid driver’s license and auto insurance Major Areas of Responsibility Provide skilled PT in patients’ homes as ordered by the attending physician Observe, record, and report patient reactions, changes, and progress Perform OASIS assessments and develop individualized care plans; adjust as needed Complete initial evaluations within 48–72 hours of referral; develop plan of care Promote continuity of care with admissions, transfers, and discharges Counsel patients and families on disease/injury management and involve them in care planning Instruct on correct use of equipment (wheelchairs, braces, walkers, etc.) and discuss financial responsibilities Educate on preventative measures to reduce rehospitalization Supervise PTAs and nurse assistants; evaluate and adjust the plan of care Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines Perform chart audits as requested; maintain licensing; attend on‑call requirements, case conferences, trainings, orientation, and professional meetings Maintain a professional appearance and follow agency policies and procedures Essential Functions Drive to patients’ primary locations per scheduled visit Daily attendance at assigned visit locations; document visits within 24–48 hours as required Establish and follow plan of care within the scope of practice for a licensed PT Health Qualifications Meet all physical requirements; maintain a negative tuberculin skin test or annual TB testing after orientation Be free of medical conditions limiting job functions; safe handling of bloodborne pathogens Physical demands: reading, speaking, writing, hearing, seeing, mobile, driving, lifting up to 50 lbs., occasional stretching, bending, reaching, and standing #J-18808-Ljbffr
