Social Worker

Bishop Rehabilitation & Nursing Center in Syracuse, NY seeks a compassionate Social Worker to support residents and families in our skilled nursing and rehab setting. The Social Worker will complete psychosocial assessments, coordinate discharge and care plans, provide counseling and crisis intervention, and connect residents with community resources. This role collaborates closely with nursing, therapy, and medical teams to promote resident-centered care, advocate for patient rights, and support transitions of care while contributing to a respectful, team-focused environment with opportunities for growth.
