Priority Care Specialist

The Priority Care Specialist primary responsibility is to provide direct in-home care services for clients that may be new, need a fill-in, or have varied diagnoses and needs that require an experienced caregiver, as well as supporting and onboarding new caregivers.

The Priority Care Specialist will be assigned required days and hours of availability for their branch that they must be available to accept client shifts. The Priority Care Specialist will receive at least 2 hours’ notice of a pending shift within their hours of availability in order for the shift to be required for them to accept.

The Priority Care Specialist will be guaranteed at least 30 hours of work per week if these requirements are met. If the Priority Care Specialist does not receive at least 30 work hours per week and the above requirements are met, they will be paid for any remaining gap in hours up to their 30 hour per week guarantee.

Schedule/Openings:
PCS 1 - Monday through Friday 7a-5p
PCS 2 - Monday through Friday 8a-6p
PCS 3 - Tuesday through Saturday 8a-6p

Pay: $27.75 per hour

Duties and responsibilities

• Provide client care as scheduled for new, emergency, or fill-in assignments.
• Establish a positive relationship with the client care team and Centralized Scheduling to facilitate good communication and awareness of each other’s needs.
• Establish a positive relationship with clients, client’s representatives, and client’s family to facilitate good communication.
• Observe and report changes, incidents, concerns, and other important information to the supervisors by phone or in client notes.
• Ensure care is provided by direction of the client’s care plan. Care tasks may include (but are not limited to):
• Performing general household chores (i.e. bed making, linen changing, dusting, vacuuming, dishes, etc.).
• Assist clients with personal care tasks (i.e. bathing, incontinence care, transfers, ambulation, etc.)
• Assist client with activities of daily living (i.e. shaving, oral care, hair care, dressing, etc.)
• Assist with meal planning and preparation.
• Transport clients to appointments, errands, etc.
• Perform tasks while following standard precautions and infection control.
• Provide training for new Caregivers as needed.
• Adhere to all FRHC policies, procedures, and state and federal regulations.

Requirements

• 18 years of age or older
• Active CPR
• Ability to pass a state and national background check
• Valid driver’s license, auto insurance and clean driving record
• One year experience in home care, AFH, hospital or nursing home setting
• Ability to lift to 50lbs
• Smart phone with ability to download and utilize the AxisCare app
• WA Only – Current valid CEs if you are an HCA

Preferred Qualifications:

• NAC, HCA, or other state approved licenses.
• Nurse Delegation Certificate
• Active state NAC/CNA or HCA license
