Staff - Registered Nurse (RN) - Hospice - $69K-90K per year

What You’ll Do as a Hospice RN / RN Case Manager Serve as the primary point of contact for patients and families throughout their hospice journey. Deliver and document skilled nursing care based on each patient’s individualized Plan of Care in their home or other setting. Perform initial and ongoing assessments to monitor condition and adjust care as needed. Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers and chaplains—to build and refine care plans. Provide emotional, physical, and spiritual support to patients and their loved ones, offering education, comfort and bereavement guidance. Teach caregivers safe care practices and provide practical training. Communicate important updates in patient condition to the care team for timely interventions. Maintain accurate clinical documentation and follow infection control and compliance standards. Support quality improvement and scheduling initiatives to ensure consistent, compassionate care. Help patients and families navigate medication administration, hospice philosophy, symptom and pain management, and end‑of‑life processes. Essential Functions Perform comprehensive patient assessments and reassessments, screen patients, and provide education to patients, families and caregivers. Deliver skilled nursing interventions and symptom management aligned with goals of care. Develop and update individualized plans of care, including visit frequency and risk identification. Coordinate care across interdisciplinary teams and consult with physicians and other providers. Provide education regarding disease process, self‑care techniques, end‑of‑life care, medications, nutrition and dietary needs. Communicate changes in condition or signs of decline to appropriate providers. Support continuity of care by recognizing appropriate referrals and transitions in levels of care. Document timely clinical records, including screening and assessment findings, physician orders, progress notes and care plans. Maintain effective working relationships and involve the team when the care plan requires coordination. Participate in on‑call and patient support activities, providing responsive clinical services. Hospice or palliative care experience preferred. Licenses & Certifications: Current RN license in the state of employment; CPR certification; valid driver’s license and automobile insurance. Specialized Knowledge and Skills: Knowledge of terminally ill patients and families, hospice services, interdisciplinary roles, assessment skills, community resources, communication, documentation, nursing practice, managed care principles and regulatory guidelines such as Medicare and Medicaid. Working Conditions: Patient homes or institutional settings; potential exposure to infections, communicable diseases, hazardous materials. Personal Traits: Sensitivity to life‑and‑death issues, tact, flexibility, effective communication across socioeconomic backgrounds, teamwork, professionalism. Equipment and Tools: Ability to operate nursing assessment tools, computers, fax, copier, cell phone or beeper; reliable transportation for work.
