Washington Avenue - Racine County, 6216 Washington Avenue Suite 200, Mount Pleasant, Wisconsin, United States of America Job Description Posted Monday, June 8, 2026 at 6:00 AM RN Care/Case Manager, Full-Time Community Care is offering a $8,000 Sign-on Bonus for Full-Time RN Care/Case Manager to join the Day Shift. This is a full-time opportunity for Racine County. RN Care/Case Manager Responsibilities Collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem solving related to member appointments, transportation, and home care needs. Conduct phone and in-person assessments using decision tools and resources to identify immediate and potential health needs. Complete all required documentation and charting and assist in the development and maintenance of member records. Job Requirements Experience: Minimum 2 years of personal, clinical leadership, and/or employment experience providing care or service for a frail or elderly population required; 2 years of recent case management preferred.
