Vibrant Emotional Health in the Bronx, NY is hiring a Case Manager/Transition Facilitator to support young adults with social and emotional needs as they gain independent living skills. You will use core practices to help youths make better decisions and improve outcomes. The role includes assessments, counseling, crisis intervention, records management, and coordination with health and developmental services.
